The skin responds as it would to any foreign object: redness, swelling, throbbing pain and, often, infection. Treatment involves clearing the offending nail edge from the sulcus, reducing the spike of nail that is doing the damage, and relieving the pressure so the tissue can settle. Debris is cleared from the nail fold and a dressing is applied where the skin is broken. The appointment runs for a full hour. That is deliberate: an inflamed toe needs care, and the surrounding nails and skin are always assessed at the same time so the cause is addressed rather than just the symptom. Digging at it with clippers pushes debris deeper, tears the nail fold and reliably converts a painful nail into an infected one. An infected toe becomes hot, swollen and may discharge; at that stage medical treatment is often required. ### Overgranulation Persistent irritation produces fleshy overgrown tissue at the nail fold that bleeds easily and makes the toe far harder to treat. ### Serious complications in diabetic feet For anyone with diabetes, neuropathy or poor circulation, an ingrown toenail is not a minor irritation. As the nail grows forward, that spike travels into the nail fold instead of past it. Other contributors include naturally curved or wide nail plates, narrow or shallow footwear, sports and walking that push the toe forward repeatedly, damp feet that soften the skin of the nail fold, and previous trauma such as a dropped object or a stubbed toe that altered how the nail grows. ### Ingrown versus involuted toenails An involuted toenail curls inwards at the sides and presses on the skin without necessarily piercing it. It is painful, and hard skin builds up in the sulcus, but the skin remains intact. An ingrown toenail has actually penetrated the tissue. The two are treated differently: an involuted nail needs regular clearance of the sulcus and reduction of the nail edges; an ingrown nail needs the offending spike removing and the wound managing. If you are unsure which you have, describe the symptoms when you book and the right appointment will be arranged. ### What conservative treatment can and cannot do Conservative treatment, clearing the sulcus, reducing the nail edge, dressing the tissue and correcting cutting technique, resolves the great majority of ingrown toenails and keeps them resolved when maintained at regular intervals. Where a nail is severely and repeatedly ingrown, or where infection is entrenched, a surgical procedure such as partial nail avulsion may ultimately be needed.
